Motor Carrier Enrollment Agreement
This Motor Carrier Enrollment Agreement ("Agreement") is entered into between Military Freight Consultants LLC ("MFC") and the undersigned Carrier ("Carrier") on 10/14/2026, 6:19:38 PM.
1. Enrollment. MFC will enroll the Carrier in the MFC carrier network for military and DLA freight. The one-time enrollment fee is $2,000, payable by credit/debit card or ACH. A $1,900 discounted rate is available when paying by Zelle to Philip Nenadov at 815.690.4970.
2. Payment. Enrollment is confirmed only after MFC receives and verifies payment. The Carrier may pay by card (processed via Cocard, processing fee may apply), ACH, or Zelle (discounted rate).
3. No Guarantee. MFC is not DLA or any Government agency. Enrollment does not guarantee freight, rates, lanes, load volume, revenue, profitability, or any contract award.
4. Authorization. The undersigned represents that they are authorized to bind the Carrier and accept these terms on the Carrier's behalf.
5. Electronic Signature. By typing their full legal name below, the Carrier provides a legally binding electronic signature under the E-SIGN Act and UETA.
